Bladder Suspension - Incontinence (Burch)

 


Open bladder and urethral surgeries are usually performed to prevent urine leakage associated with stress incontinence. Stress incontinence is an involuntary leakage of urine that occurs when laughing, coughing, sneezing, or lifting. The condition can be caused by deformity or damage to the urethra, bladder, or pelvic muscles. Multiple births and menopause can cause a loss of muscle tone in the bladder area.

The bladder may sag into or outside the vagina. You may feel this during sexual intercourse, or may even see the bladder protruding outside of the vagina.





 
Causes

It is caused by weakness or deficiency in the muscles supporting the urethra. This allows urine to leak out of the bladder unexpectedly when there is increased abdominal pressure from physical activities such as standing, lifting, exercising, and even coughing or sneezing.

The best candidates

Like in any other surgical procedure, have a good health condition define weather a patient qualifies or not for surgery. Not all the patients qualify, therefore it is important to perform some tests before surgery in order to define if you qualify for this procedure benefits.

 

Common complications

The possible complications are the same as in any other surgery: hematomas, bleeding, swelling, etc. But they get corrected within some days after surgery. In some cases there can be vaginal or urethral lesions which can be fixed on the same surgery or even on a second surgical procedure.

Your sugery

The purpose of the surgery is to try to return the bladder and urethra to its normal position. There are two common ways of performing stress incontinence surgery: through the abdominal wall or though the vagina. Surgery may either require general anesthesia, local anesthesia, or regional (spinal or epidural) anesthesia. Generaly the surgery takes about 1 hr. It is usualy performed under spinal anesthesia and some times you can leave the clinic the same day. For the patients safety it is sometimes recomendable to leave the Catheter in the bladder for at least 48 hrs.

After your surgery

The level of succes of the surgery depends also on the level of the patient’s problem; sometimes it is not so important to be absolutely l dry, but to improve the life of quality and being able to perform all daily activities.

Getting back to normal

Most of the patients that go throu this procedure can go back to school or work after one or two weeks after surgery.
